Who does not want a flawless pimple free skin. But sometimes due to some reasons, we get skin disease or allergies in the form of acne and pimples and sometimes even eczema and psoriasis.
Due to consumption of hot, spicy or fried food, stressful lifestyle, hormonal changes and genetic reasons many of us often suffer from acne-pimples. Extremely poor diet and extremely bad lifestyle sometimes contaminates our blood to the extent that we fall prey to serious diseases like eczema and psoriasis. The impurities in blood is the reason for skin allergies also.
Today, we will talk about the diseases of skin like acne, pimples, eczema and psoriasis. Along with this, we will also discuss skin allergies. As usual we have with us a Senior Ayurveda Expert, Dr. Parmeshwar Arora, in our studio. We welcome you sir, to the show.
Q. Acne-pimples happen to be a big problem amongst girls. First of all, please tell us that why do we get these white heads, black heads, acne or pimples?

A. If you want to know the basic reason behind these, then I think first of all you will have to understand an important structure called sebaceous glands. If we talk about the anatomy of sebaceous glands then these are connected to hair follicle. Together, they are known as pilo-sebaceous unit. The creatin of hair follicle and sebum of sebaceous gland come out through pilo-sebaceous duct, on the surface of skin. While creatin helps in hair formation, sebum keeps the skin oily. But if for any reason the sebum gets blocked inside the pilo-sebaceous duct then an inflammatory process gets started. It gets affected by bacteria and somehow the process of decomposition called putrefaction starts. This comes out in form of comedones which we also call as white heads and black heads or papioles or pustules or cystic lesions or sometimes in the form of nodules also. So if we have to understand the basic nature of acne then it starts with blockade of sebum in the pilo-sebaceous duct.
Q. Somehow, it acts like an obstruction in the age which is believed to be the most crucial in one’s life i.e. the young age. This is the age when a boy or a girl wants to look his/her best but this is when acne and pimples trouble the most So, what should be done?
A. True, problem of acne and pimples is at its peak in the stage of life when most of the people want their skin to look clear and flawless.. To explain the reason behind it, I will again take you to sebaceous glands. Although these sebceous glands are present since our birth itself but it is observed that in the first decade of life they are small and remain inactive. But, as soon as we enter the second decade of our life they undergo activation and stimulation. Starting from middle of second decade till middle of third decade i.e. from 15 years of age to 25 years of age, these glands show maximum stimulation. In another words, maximum secretion of sebum from sebaceous glands happen in the phase between 15 years of age till 25 years of age. Now to tell you about the cause of increased stimulation in this phase of life, this stimulation is dependent on hormone, up to a great extent. From the hormones, specifically endrogen enhances the stimulation of sebaceous glands. If there is any carelessness during this age like if we do not keep our skin clean then the chemicals or pollutants obstruct the outward flow of sebum in pilo-sebaceous duct. There are some conditions too when the sebaceous glands become hyperactive and hence the secretion of sebum increases much more than in normal conditions. In both the conditions, acne, black heads, white heads etc. can develop on the skin. Here I would like to explain what exactly are white heads ad black heads. As I said, secretion of sebaceous glands reaches out to the surface of skin through pilo-sebaceous glands. If the creatin and sebum create obstruction or plugging in the superficial layer i.e. close to surface of skin then the inflammatory product which is decomposed and clogged substance is formed near the skin and this is what we know as black heads. Open comedones are black heads. Against this, if the product of creatin and sebum is formed deeper in pilo-sebaceous duct then it does not come out. This forms white heads i.e. open comedones.
Q. You have explained so nicely that what actually are white heads and black heads. Now we would also like to understand that why do variations in sebum secretion happen or in other words what causes disturbances in sebaceous glands?
A. As I explained earlier, there could be two reasons for this. Firstly, the sebum gets stuck in the pilo-sebaceous duct and secondly, there is increased production of sebum (this condition is termed as seborrhea. If you want to understand the situations which cause seborrhea then primarily it is any type of hormonal imbalance in the body. The balance between estrogen and endrogen has a vital role in stimulation of sebaceous glands. If there is any disturbance caused in this balance or if there is increase in release of endrogen (a hormone released from testicles in male, in small amount from ovaries in females and also from adrenal glands) then it enhances the stimulation of sebaceous glands to a great extent. This results in an unexpected increase in production of sebum and this is condition which we call as seborrhea. Apart from this dietary reasons are another important factor in causing seborrhea. We observe that intake of hot/spicy, fried or sour items or any thing which disturbs the digestion, can be a cause of enhanced stimulation of sebaceous glands. Thirdly comes the psycogenic factors which involve mental disturbnces also cause the condition of seborrhea. Additionally, some genetic factors also cause the condition of seborrhea. Some drugs like corticosteroids, endrogen steroids, anti-epileptic medicines also cause the condition of seborrhea. Ingestion of halogens and many other reasons and also some diseases can cause the condition of seborrhea. This was about the condition when we somehow, have increased the production of sebum.
Another situation which is seen most often is when our pilo-sebaceous duct gets obstructed. The reason for this is, while we frequently apply makeup but we do not clean them completely. Also, we get eposure to dust and various pollutants in the environment, but we again don’t clean our skin properly.
Thus, if you have either of the condition i.e. seborrhea or obstruction in pilo-sebaceous duct, then it can lead to acne-pimples.
Q. Perhaps, acne have various types too. What are its different types?
A. Sometimes, we term the acne depending upon its form. E.g. if it is in the form of papules, we call it acne-papulosa, if it is in the form of pustules we call it acne-pustulosa. Similarly, sometimes, the name is given according to its position. E.g. if it is present on eyelids or its edges, we name it acne ciliaris, if it is present on the corners of mouth, it is called acne cratose. Also, sometimes the name is according to severity of condition. E.g. Acne fulminans, wherein the acne is quite inflamed, tender and with ulceration. This is usually seen in teenagers and is very acute in nature. It may sometimes involve fever and joint pains. The most common type of acne is acne vulgaris. One more type is acne tropicalis which happen due to hot and humid weather conditions. But according to me the term or the name is not of much importance because when we go to the treatment, the principle does not have any difference and it remains the same for all types. So, I would say or atleast according to me, all acnes are similar.
Q. Does the treatment have any factor of age, involved?
A. As I told you earlier, there is a bracket of age when the sebaceous glands are most active. This is between 15 years of age to 25 years of age, that the sebaceous glands are most active and there is maximum secretion of sebum. Unfortunately, this is also the phase when we go out maximum, we party the most and eat maximum junk items. All such things, aggravate our pitta and our vayu and contaminate our hormones too. So its somehow unfortunate that the age when we commit maximum violations in terms of our diet and lifestyle, is also the age when there is maximum secretion of sebum happening. Thus there are increased chances of being trapped.
Q. This informs us that we need to take special care of our diet too, if we want our skin to remain flawless and free from acne-pimples or any other skin diseases. So many useful informations but now I would like to know about its treatment. What kind of solution do you provide to encounter the problem of acne-pimples?
A. Those who want to get rid of their acne-pimples would have to pay attention to the reasons behind the problem. Simpler reason for acne-pimples is plugging up of pores of skin. To avoid this, you need to keep your skin clean. For cleaning we have very simple remedies available. We ask our patients to rub papaya or tomato. If you make it a habit to apply gulab jal (i.e. rosewater) with few drops of lemon juice in it and rinsing it off with cold water then the pores will remain open and you would be able to avoid the pimples which result due to clogged pores. Lemon is a well known shrotoshodhak (cleanser) and rosewater is a very good hydrator. So they will help you in avoiding black heads, white heads, pustules, papules etc. Furthermore, you can use apricot as a scrubber. So, basically our objective should be to keep our skin clean. I would like to specially tell it to our women readers since the make-up has become mandatory. Do make-up if it is required but I would still suggest it that avoid the use of chemicals on your skin. There are many natural alternatives available. And keep smiling, that enhances your beauty manifold. I somehow don’t like that people use a lot of make-up but I don’t know why, they limit their smiling. Jokes apart, limit the use of chemicals on your face and keep your skin clean. Don’t forget to clean your face off make-up before going to bed. In case of boys since they go out a lot, they too are exposed to various pollutants present in the atmosphere. These pollutants clog their pores. So we all should make it a habit to clean your face every night with rosewater containing few drops of lemon juice and rinsing it off with water after some time. This will ensure that there is no obstruction of sebum in the layers of skin and thus the acne resulting from putrefaction would not happen any more.
Coming to the internal medication, if we go for allopathic treatment you get a remedy in the form of anti-biotics which you have to take for a long period of time. According to me, this is not the right kind of approach, at all. It only works on the putrefaction in pilo-sebaceous glands but rest of the pathologies like hormonal imbalance, dietary imbalance, aggravation of pitta and impurity in blood, remain unattended.
Q. Does that mean, allopathic treatment does not completely cure pimples?
A. Of course! When you take anti-biotics for a long period of time, the acne-pimples disappear but somehow your digestion gets disturbed and you get other associated problems. So, it is not definitely, a right approach. See, the problem of acne pimples is such that sometimes people become quite sensitive. Sometimes when the patients approach me with problem of skin as well as digestion, I tell them that let me treat one of the problems first. 99% of such patients chose skin first. So you can see how important their skin is.
I first thing that I ask all the skin patients, irrespective of their problem is they need purification of blood. Secondly, their pitta needs to be suppressed, Thirdly, their vayu needs to be regulated, Fourthly, crimy i.e. microbes which could be present in their blood would need to be removed. After that, to kill baacteria we would need to start an anti-biotic type action. At last but very importantly the blood circulation would need to be improved. This prevents the dryness on skin. Now I would repeat the six steps that a patient of any skin disease would have to follow:
- purification of blood
- suppression of pitta dosha
- regulation of vayu
- removal of microbes or krimi from blood
- give anti-biotic effect with the help of selected herbal medicines
- increase blood circulation
If you achieve all these six things, you will definitely enjoy a flawless skin.

A. Obviously, I would like to announce the name of this sixer. It is Suderma. A. Irrespective of whichever skin disease it is, when we go to their root cause it is the same. Just to make you understand, I will give you an example. See, all of us know that we have dandruff on our scalp. When we run a comb or fingers through scalp, a white powdery substance fall out of it and this is what we call dandruff. When this dandruff is in wet form, we call it seborrheic dermatitis. When seborrheic dermatitis is in the form of plaque i.e. large thick pieces, we call it psoriasis. As you see, dandruff, seborrheic dermatitis, psoriasis, they are different names but infact three forms of the same problem. Similarly, if we talk about eczema and dermatitis then both complement each other. It is said that any type of skin disease i.e. dermatitis, if left unattended, progresses to eczema. Eczema has two forms, atopic dermatitis and seborrheic dermatitis. Seborrheic dermatitis when acquires severity, it looks like psoriasis. So, though they have different names but their basic pathology can be common. They could be because of hormonal reasons, genetic reasons, impurity in blood, aggravated pitta dosh or vata prakopa. Because of dominance of one reason above other, the symptoms vary and accordingly their names vary but if we observe the cumulative reasons then all of them would be there with only difference in their order. What I mean to say is, let whatever be the name, but if we attend all the listed six problems any skin problem will get resolved from the root. If you leave any one of the reason unattended, like if you miss the stress aspect in the treatment, the problem would not be cured. If for the psoriasis patient, eczema patient or dandruff patient whose skin becomes dry, you do not correct the blood circulation, the problem would not be cured. Considering all the aspects and after conducting a vast research work, we gathered all the factors that needed to be covered so that the patients of all kinds of skin diseases could be provided with a complete cure. Q. Very true! That’s quite a successful effort and Suderma is doing its job well. One important thing, how should one take this medicine?
A. This is in capsule form. We have taken care about the palatabilty as well as convenience in carrying the medicine. That’s why it is in capsule form. This medicine includes, neem, manjishtha, amaltas, harar, gandhak rasayan, rasamanikya etc. To remove crimy, we included vidang. So, this is the kind of combination, that we have used in this medicine. Suderma needs to be taken as 2 capsules thrice a day: 2 capsules after breakfast, followed by 2-2 capsules each before lunch and dinner.
